After reading various threads on here, I am very confused. Do we have to fill the above form in if we are fully vaccinated ?
If we do then please would you be able to explain how we scan the QR code from our paper copies.

Yes you do. If you use a smart phone you can do it with the qr code reader from your ada pass or NHS. If not use a computer and follow the instructions. You do not have to download them if you can't just take them with you.

Only other thing I would add is that of course if you fly into the North the Guv app is all you required but if you fly into the South you will require that and whatever the South require. Guv app can only be completed 2 or 3 days before travel and if it is some time before you fly best to check on the latest and greatest rules at that time. Sometimes here it is difficult to keep up with it all.

We completed our form before we left for the UK, spent 10 days there no problem on return.

That is weird because when I tried to complete it the first time it said it was only valid up to a date about 3 days from the application. We flew back on the 26th applied on the 24th and it expired on the 27th.
